Intake Appointments
Intake appointments are crucial for:
- Making a client feel comfortable and heard
- Establishing therapeutic alliance
- Evaluating whether this is the right fit
- Presenting a professional service
- Outlining ethical responsibilities
- Establishing goals and roadmaps for treatment
Depending on your client's age, we recommend having a parent session for the intake appointment.
Preparation for an Intake Appointment
If a client has submitted their forms appropriately, it is important that the clinician gives the client confidence that they have read the information and are prepared for the session.
- Read the client files and submitted consent forms before the intake session to familiarise yourself with the client and their therapy goals
- Check the Referrer section to see if the client has been referred by a medical practitioner (you will need to check validity of MHCP referral if applicable)
- Take note (in your notability for the session) of age/grade, suburb and family member names if specified
- Write down any specific information from the consent form that may be helpful to reference
- Write down any specific questions you may have for the client based on the outlined referral reason
If the client has not completed necessary forms
- Ask reception to contact the client to remind them to complete the forms
- If not completed at all, remind the client during the session that you have limited information regarding background or reason for presentation, so more time will need to be spent during session on this
- Remind client they will still need to complete the forms and can use an iPad in the waiting room before they leave if they convenient
Completing Paperwork After Intake

- Mental Status Examination (if applicable by clinicians’ judgement)
- MSE can be a helpful assessment tool on intake and can allow for accurate tracking over the course of therapy.
